NOTICE OF TRANSFER OF PATIENT RECORDS
On April 19, 2022, the assets of The Health Depot Pharmacy (“Health Depot”) were transferred to 193602 Canada Inc. (“Purchaser Pharmacy”), which is an affiliate of Green Shield Holdings Inc. and is a part of the Green Shield Canada group of companies. This transfer was part of the planned transition of The Health Depot’s pharmacy operations to the Purchaser Pharmacy.
As part of the above transfer, The Health Depot transferred all its patient records, including your health records, to the Purchaser Pharmacy. The Health Depot undertook various measures to ensure that the health records were transferred in compliance with its obligations under the Personal Health Information Protection Act, 2004 (“PHIPA”). As the new custodian of your health records, the Purchaser Pharmacy is, like The Health Depot, required to collect, use, disclose, retain and dispose of your health records in compliance with PHIPA.
